1.	It is agreed that you are enrolling in Driversity's driver training course.
2.	It is agreed that a non refundable administrative enrollment fee for enrollment is assessed. The fee for a new student is $35. A $20 fee is charged for a returning Driversity, LLC, or I-Virtual High student.
3.	It is agreed that you may have the certificate sent to you or you may pick the certificate up from the Antioch office between 12pm and 4pm M-F.
4.	You are acknowledge that you are within the range of the ANTIOCH location. *If you are not see the note above at the top of this form.*
5.	You understand that once the certificate is issued the fee is not refundable for any reason.
